Discharge Planner
Job Summary: The candidate works closely with the case management team and assists in discharge planning services for patients admitted to Westchester Medical Center. They are a part of the multidisciplinary treatment team and be able to function independently on a professional level. The candidate must possess excellent oral and written skills, be organized and possess strong computer skills. Responsibilities: Actively participate in multidisciplinary team rounds. Interacts with treatment team, patients, families, and community agencies in developing and arranging individualized discharge plans.
