Salary: $49.499 - $67.709 Hourly
Salary is commensurate with years of RN experience indicated at time of application submittal. Experience not disclosed or documented at the time of application will not be considered for initial step placement.
JOB SUMMARY UNION - NNOC
The APN-Nurse Practitioner provides care to People Living with HIV (PLWH) and persons at risk for HIV supporting the Cook County Health Integrated Program (CCHIP) at Provident Hospital of Cook County Health and throughout Ambulatory & Community Health Network (ACHN) locations as needed.
TYPICAL DUTIES
Provides primary care services to PLWH in clinics at CORE Center and other ACHN clinic sites through appointments, telephone follow-up, patient interviews, health maintenance counseling, compliance/exit interviews, liaison with diagnostic services, patient advocacy, and referrals to case managers and chemical dependency counselors as warranted.
Provides sexually transmitted infection (STI) assessments, evaluation, and treatment and treatment as well as Pre-Exposure prophylaxis (PrEP) to those at risk for HIV acquisition.
Counsels patients regarding transmission of STIs/HIV and risk reduction strategies.
Provides evaluation, treatment, and counseling for viral hepatitis (hepatitis B and
hepatitis C) patients in collaboration with the attending physician and assists with
obtaining prior authorization for those services, as needed.
Elicits a comprehensive, accurate history and performs a routine physical examination
of patients who are 18 and older on initial presentation to clinic.
Elicits an accurate history and performs a problem-oriented screening examination for
patients in follow-up situations.
Selects performance and/or interprets the appropriate routine laboratory and diagnostic
studies/procedures.
Maintains records of routine laboratory and diagnostic studies/procedures for the
purpose of completing medical records and maintaining an adequate database.
Identifies problems, organizes, and integrates patient information and records and presents pertinent data.
Documents all care provided, procedures performed, and medication list in the electronic health record (EMR).
Initiates/implements a medical management plan in collaboration with the supervising physician.
Instructs and counsels patients regarding physical and mental health to include diet,
health maintenance, therapy, referral, and follow-up services.
Reviews and monitors medical management and therapy plans and implements
appropriate continuing treatment in collaboration with supervising physician.
Cooperates with support services personnel to implement the social service treatment
plan in conjunction with the interdisciplinary team.
Participates in continuing education in HIV to remain current.
Maintains strict confidentiality regarding patient information.
Performs duties in keeping with CCHIP policies and procedures.
Attends routine meetings to discuss HIV and CCHIP Programs and activities.
Performs other duties as assigned.
