DescriptionSummary:As Patient Navigator / Health Educator, the associate will assess the education needs of workplace wellness partners. Design programs to address those needs and meet workers' varied schedules, with specific implementation plans, evaluation, and achievement of goals and measures. Align associate educational programs with overall goals of hospital and workplace wellness partners. Associates will also provide Navigation to educate and provide outreach to communities and high-risk populations. This individual provides support and advocates for those in the community who need extra help. This individual does not provide patient care and does not extend or substitute for the more specialized services of a doctor, nurse, or social wo
